Fill out your intake form at least 24 hours before your appointment. The sooner, the
better!

Dive into all the resources waiting for you, including access to our exclusive AZ Elite
Medical Care Facebook group.

5. Meet with Eddie, Our Nurse Practitioner
During your scheduled consultation, Eddie will help craft your personalized plan.

6. Create Your Customized Plan
We’ll work together to build a plan that’s as unique as you are!

7. Put Your Plan into Action
Use the resources and tap into our community for all the support you need along the way.

8. Receive Your Medication
Your medication will arrive at your doorstep within 3-5 days of payment. Easy peasy!

9. Watch Our How-To Video
Head to your Client Portal for a video tutorial on medication dosing and
administration.

10. Expect a Check-In
We’ll touch base with you during the first week to see how things are going.

11. Monthly Follow-Up
Your first follow-up will be one month after starting, with regular monthly check-ins to keep you on track.
